Why does a new TV licence sometimes not last 12 months when you've paid for a whole year?

You pay from the beginning of the month you are applying for the licence so if you applied on 15th of Feb you'd be paying for Feb 1st-14th as well and the licence will expire at the end of next Feb 2012.
